THE BASIC REQUIREMENT FOR THIS FORM
STEMS FROM THE DISASTER RELIEF ACT OF 1974, PUBLIC LAW 93-288, AND
FEDERAL DISASTER ASSISTANCE REGULATION (44 CFR PART 205). FEMA
POLICIES AND GUIDELINES (P.L. 93-288 AND P.L. 93-234) REQUIRE UNDER
CERTAIN CONDITIONS THE APPLICANT MUST OBTAI AND MAINTAIN HAZARD
INSURANCE AS A CONDITION FOR RECEIVING DISASTER ASSISTANCE. THIS
FORM DOCUMENTS THE INSURANCE COMMITMENT AND CAN BE USED FOR FLOOD
INSURANCE AS WELL AS OTHER TYPES.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
