UNDER THE ACT, MAY HAVE BENEFITS PAID DIRECTLY, IN LIEU OF PAYMENT
ON HIS OR HER BEHALF, TO A LEGAL GUARDIAN OR REPRESENTATIVE PAYEE.
THE CERTIFICATE WILL OBTAIN, FROM THE MINOR, INFORMATION ON
SCHOOLING, WORK AND USE TO BE MADE OF THE BENEFITS. THE INFORMATION
WILL BE USED TO DETERMINE IF THE BENEFITS SHOULD BE PAID DIRECTLY
TO THE MINOR.
