Abstract: FORM ETA 6-57 IS AN INTEGRAL PART OF THE SCREENING AND ADMISSIONS FOR ENROLLMENT IN JOB CORPS. THE REGIONAL OFFICE NOTIFIES THE SCREENER OF ELIGIBILITY AND CENTER OF ASSIGNMENT FOR THE YOUTH. IT THE YOUTH ACCEPTS, THE ETA 6-57 IS COMPLETED AND ACCOMPANIES THE YOUTH TO THE CENTER. IF YOUTH REFUSES THE ASSIGNMENT, OR DOES NOT SHOW FOR DEPARTURE, THE FORM IS USED AS MEANS OF COLLECTING DATA ON REASONS FOR THE YOUTH'S NOT SHOWING
