Abstract: THIS APPLICATION FORM IS USED BY LARGE NEW INDUSTRIAL FACILITIES AND LARGE MUNICIPAL SOURCES. APPLICANTS MUST PROVIDE INFORMATION TO IDENTIFY AND ANALYZE THE POLLUTANT LOADINGS OF THE EFFLUENT STREAMS. THIS INFORMATION IS USED BY PERMIT WRITERS IN ORDER TO DEVELOP PERMIT CONDITIONS APPROPRIATE FOR EACH SPECIFIC FACILITY.
