Abstract: THIS QUESTIONNAIRE IS COMPLETED BY HEALTH PROFESSIONALS WHO HAVE INCURRED A SERVICE OBLIGATION THROUGH THE NATIONAL HEALTH SERVICE CORPS (NHSC) SCHOLARSHIP PROGRAM. IT IS USED TO ASSIST THE NHSC CENTRAL AND REGIONAL OFFICES IN MATCHING SCHOLARSHIP RECEIPIENTS TO HEALTH MANPOWER SHORTAGE AREAS IN ORDER TO FULFILL THEIR SERVICE OBLIGATION.
