Abstract: THIS FORM WILL PROVIDE CSA WITH A RELATIVELY COMPLETE LISTING OF THE CAA'S ENTIRE WORK PROGRAM FOR A TWO-YEAR PERIOD. ALL PROJECTS ADMINISTERED BY THE CAA AND REQUIRING ANY CSA MONEY WILL BE LISTED ON THE FRONT SIDE OF THE FORM. FOR ACH PROJECT LISTED THE CAA WILL GIVE INFORMATION ON PROJECT COSTS AS WELL AS LIST THE STANDARD EFFECTIVE NESS--THROUGH WHICH THE PROJCT IS INTENDED TO ACHIEVE THE PURPOSES OF A COMMUNITY ACTION PROGRAM
