Abstract: THE INFORMATION IS NEEDED TO INFORM THE COAST GUARD THAT A VESSEL OR PERSONNEL ACCIDENT HAS OCCURRED. THIS INFORMATION IS THEN USED BY THE COAST GUARD TO INITIATE AN INVESTIGATION AS REQUIRED BY 46 USC 239. SUBMISSION FULFILLS THE PUBLIC REPORTING REQUIREMENTS OF 33 USC AND 43 USC 1348.
