Abstract: SECTION 202(I) OF THE SOCIAL SECURITY ACT PROVIDES FOR THE PAYMENT OF LUMP-SUM DEATH BENEFITS TO INDIVIDUALS WHO QUALIFY. THIS FORM IS USED BY PERSONS REQUESTING THE LUMP-SUM DEATH PAYMENT ON BEHALF OF A NONADMINISTERED ESTATE.
