Abstract: THIS FORM IS USED TO MONITOR THE ACTIVITIES OF THE VOLUNTEER AT THE VITA/TCE SITE TO ENSURE THAT QUALITY SERVICE IS BEING GIVEN. SITE VISITS WILL ENSURE THAT VITA VOLUNTEERS AND TCE SPONSORS ARE COMPLYING WITH PROGRAM GUIDELINES. VISITATION CHECKSHEETS SHOULD BE AVAILABLE FOR REGIONAL AND NATIONAL OFFICE REVIEW.
