Abstract: USED TO CARRY OUT THE REQUIREMENTS OF TITLE IV, SECTION 433(A) OF CETA THE FORM IS THE BASIC APPLICATION AND INFORMATION DOCUMENT USED IN THE SCREENING AND SELECTION OF APPLICANTS. ITS PURPOSE IS (1) TO SERVE AS BASIC APPLICATION FORM, (2) TO DETERMINE WHETHER YOUTH MEETS ELIGIBILI CRITERIA, AND (3) PROVIDE DEMOGRAPHIC DATA FOR PROGRAM EVALUATION.
