Abstract: IN ORDER FOR A PLAN SPONSOR TO TERMINATE A DEFINED BENEFIT PLAN COVERED UNDER TITLE IV OF ERISA, PBGC NEEDS CERTAIN INFORMATION WHICH ALLOWS IT TO DETERMINE WHETHER THE PLAN WILL REQUIRE PBGC GUARANTEES, AND IF SO, HOW MUCH. THIS FORM GIVES PBGC THE NECESSARY INFORMATION.
