Abstract: THIS FORM WILL BE USED BY 500 VOLUNTEERS TO EVALUATE THE NEW VITA COUR MATERIAL. IT ALSO PERMITS IRS TO GIVE FEEDBACK TO THE VOLUNTEER ON THEIR ATTAINMENT OF THE LESSONS. THIS IS A ONE-TIME EVALUATION.
