included on DD Form 214, Report of Separation from Active Duty,
which is required as an attachment to educational assistance
applications (e.g., date entered/separated from active duty, type
of separation or discharge, branch of service). This discussion
should include an estimate of the percentage of instances in which
a DD 214 is not attached to an application.
