Abstract: THE AGENT ORANGE REGISTRY CODE SHEET WILL REPLACE THE CODE SHEET PREVIOUSLY USED. THE REVISED CODE SHEET IS MORE CONCISE ND WILL INVOLVE LESS TIME TO COMPLETE BY VA MAS PERSONNEL. THE CODE SHEET WIL BE USED BY VA PERSONNEL AND THE INFORMATION WILL BE OBTAINED (FOR INPUT ONTO THE CODE SHEET) THROUGH THE PERSONAL INTERVIEW PROCESS DURI THE EXAMINATION.
