THE PURPOSE OF THE OUTPATIENT PHYSICAL
THERAPY/SPEECH PATHOLOGY SURVEY IS TO DETERMINE IF OUTPATIENT
PROVIDERS OF PHYSCIAL THERAPY AND SPEECH PATHOLOGY PROVIDERS
INITIALLY AND/OR CONTINUE TO MEET MEDICARE REQUIREMENTS FOR PROGRAM
PARTICIPATION.
