Abstract: AN APPLICANT FOR A U.S. PASSPORT IS REQUIRED BY STATUTE (22 USC 213) TO SUBSCRIBE TO AND SUBMIT A WRITTEN APPLICATION. FORM OF-178 IS THE APPLICATION USEDBY OUR FOREIGN SERVICE POSTS FOR THIS PURPOSE. IT SOLICITS THE INFORMATION NEEDED TO ESTABLISH THE APPLICANT'S IDENTIFY AND CITIZENSHIP. IT IS ALSO USED AS A REGISTRATION APPLICATI (A PROCEDURE WHEREBY A U.S. CITIZEN'S PRESENCE IN THE CONSULAR DISTRIC IS MADE A MATTER OF RECORD AT THE CONSULAR POST).
