CARRIERS NOT STARTING SERVICE OR NOT OPERATING UNDER AUTHORITY FOR WHICH A FITNESS DETERMINATION HAS BEEN MADE FOR 2 YEARS AFTER FITNESS FINDING MUST RE-FILE DATA ABOUT THEIR FITNESS.
