Abstract: THE LAW REQUIRES ALL PERSONS TO OBTAIN A PERMIT BEFORE DEALING IN SPECIALLY DENATURED SPIRITS. APPLICATION FOR THE PERMIT IS MADE ON THIS FORM. THE APPLICATION REQUESTS CERTAIN INFORMATION SUCH AS LOCATION OF PREMISES AND STORAGE CAPACITIES. THIS INFORMATION IS USED TO MAKE A DETERMINATION AS TO WHETHER OR NOT THE APPLICATION QUALIFIES FOR A PERMIT.
