A COMPUTER-GENERATED EQUIVALENT MAY BE USED IN LIE OF FORM 742 FOR REPORTING OF MATERIAL BALANCE DATA PROVIDED IT CONTAIN ALL INFORMATION NECESSARY FOR PROPER REPORTING OF MATERIAL BALANCES, AND HAS BEEN APPROVED BY THE COGNIZANT FIELD ORGANIZATION.
