Abstract: FORM IS TO BE USED TO LOG TELEPHONE AND WRITTEN REQUESTS FOR EXPORT INFORMATION RESULTING FROM SBA'S PUBLIC INFORMATION MEDIA CAMPAIGN DIRECTED TO SMALL AND NEW EXPORTERS.
