INFORMATION REQUIREMENTS ARE (1) OBTAINING WRITTEN PATIENT CONSENT, (2) DOCUMENTING "MEDICAL PERSONNEL" STATUS OF RECIPIENT OF A DISCLOSURE TO MEET A MEDICAL EMERGENCY, AND (3) NOTIFYING EACH PATIENT OF FEDERAL REQUIREMENTS.
