This collection
is disapproved in accordance with 5 CFR 1320.4(b)(1). It is OMB's
understanding that the data collection necessary to support the
MHSP evaluation is now completed. Since the HCFA 255 incorporates
data elements designed for evaluation purposes, the HCFA 255 is
more burdensome than required for cost reimbursement purposes.

COST REPORT FORMS FOR REPORTING MEDICARE COSTS UNDER THE WAIVER
EXPIRE APRIL 30, 1983 AND WIL BE RENEW FOR 3 MORE YEARS IN ORDER
FOR CLINICS TO REPORT AND BE REIMBRUSED ON A COST BASIS AS LONG AS
THE WAIVERS ARE IN EFFECT, THROUGH DECEMBER 1984 AT A MINIMUM, AND
POSSIBLY LONGER.
