Abstract: THIS IS THE SECOND AND FINAL ROUND OF DATA COLLECTION IN A LONGITUDINA STUDY OF FUNCTIONAL IMPAIRMENTS IN PARTICIPANTS. THIS QUASI-EXPERIMENTAL STUDY WILL DETERMINE WHICH FUNCTIONAL LEVELS OF CLIENT NEED ARE BEST SERVED BY SCP AND RSVP LONG-TERM CARE ACTIVITIES.
