Abstract: INFORMATION NEEDED TO INFORM THE COAST GUARD THAT A VESSEL OR PERSONNE ACCIDENT HAS OCCURRED. THIS INFORMATION IS THEN USED BY THE COAST GUARD TO INITIATE AN INVESTIGATION AS REQUIRED BY 46 USC 239. SUBMISSION BY VESSEL OWNER/OPERATOR FULFILLS REPORTING REQUIREMENTS OF 33 USC 361 AND 43 USC 1348.
