the form. Number and expiration date currently on form are two
years out of date. 2. Form must include a statement of why
information is being collected, how it will be used, and whether
response is mandatory, required for benefit, or voluntary, per 44
USC 3504(c)(3)(C). 3. Form should display correct name of
collecting agency. 4. All the above flaws should have been detected
and corrected by USIA before the form was submitted to OMB for
