Abstract: THIS ENROLLEE APPLICATION FORM PERMITS PUBLIC ACCESS TO EMPLOYMENT IN THE YCC PROGRAM. THE FORM IS SPECIALLY DESIGNED TO PROVIDE VERY LIMIT BUT ADEQUATE INFORMATION TO DETERMINE YOUTH ELIGIBILITY FOR THE PROGRA IN A RANDOM SELECTION PROCESS.
