Abstract: HIGH SCHOOL SCIENCE TEACHERS WILL FILL OUT THESE FORMS AS PART OF AN OVERALL CLEAN-UP PROGRAM FOR HAZARDOUS CHEMICALS IN THE SCHOOL LAB. THE FORMS ARE PART OF A DOCUMENT WARNING TEACHERS OF SUCH CHEMICALS. CPSC NEEDS THE RESULTS TO SHOW THAT HAZARDOUS CHEMICALS HAVE BEEN REMOVED FROM SCHOOL LABS AND TO EVALUATE THE EFFECTIVENESS OF THE PUBLICATION.
