Terms of Clearance: Form needs improved graphics and clearer instructions. Question 25 was considered to have possible self-incrimination implications for the respondent. Justice Dept. had concerns about questions 30-33 which have been dropped from other OPM forms.

Abstract: E.O. 10450 REQUIRES THAT ALL CIVILIANS ENTERING THE FEDERAL EXECUTIVE EXECUTIVE BRANCH BE SUBJECT TO A BACKGROUND INVESTIGATION. BACKGROUND INVESTIGATIONS ARE ALSO REQUIRED BY E.O. 10477.5 U.S.C. 3301, 42 U.S.C 2165 AND OTHER STATUTES. STANDARD FORM 86 IS COMPLETED BY FEDERAL AND NON-FEDERAL PERSONNEL FOR EMPLOYMENT AND/OR SECURITY CLEARANCES. INFORMATION COLLECTED IS USED TO INITIATE THE REQUIRED BACKGROUND INVESTIGATION.
