Abstract: THIS FORM IS USED TO DETERMINE THE AMOUNTS OF ROYALTIES PAYABLE TO THE UNITED STATES UNDER FEDERAL OIL AND GAS LEASES, AND DESIGNATE THE PERCENTAGE OF TOTAL PRODUCTION TO BE ALLOCATED TO EACH FEDERAL LEASE COMMITTED TO AND INCLUDED IN A UNIT PARTICIPATING AREA OR COMMUNITIZATION AGREEMENT.
