Abstract: THE INFORMATION SERVICE REACTION FORM IS SENT TO NCBE USERS TO HAVE THEM APPRAISE ITS SERVICES AND PERFORMANCE. THE INFORMATION IS USED TO MODIFY NCBE SERVICES. THE FORM IS SENT OUT WITH EVERY RESPONS TO INFORMATION. USERS ARE ASKED TO RETURN THE FORM ON A VOLUNTARY BASES.
