A MINERAL SURVEY OF THE MINING CLAIMS MUST BE SUBMITTED AS PART OF THE MINERAL PATENT APPLICATION. FORM 3860-5 IS AN APPLICATION FOR THE SURVEYING BY A CADASTRAL SURVEYOR. RESPONDENTS MAY RANGE FROM AN INDIVIDUAL TO A MULTI-NATIONAL CORPORATION.
