Abstract: HCFA PROVIDES FREE FORMS TO COMPUTER FIRMS THAT DO THE BILLING FOR CARRIERS AND INTERMEDIARIES. TO ASSURE THE MOST EXPEDITIOUS AND ECONOMICAL USE OF THESE FORMS, WE REQUIRE USERS TO GIVE US CURRENT INVENTORY AND PAST USAGE FIGURES AND BASE SUPPLIES SHIPPED ON THESE FIGURES.
