Abstract: THIS SURVEY WILL COLLECT INFORMATION FROM PERSONS AND ORGANIZATIONS INVOLVED WITH COMPLETION OF BIRTH, DEATH, FETAL DEATH, MARRIAGE, DIVORCE, OR INDUCED ABORTION RECORDS OR WHO USE DATA DERIVED FROM THES RECORDS. THIS INFORMATION WILL AID IN EVALUATING THE INFORMATION CURRENTLY BEING COLLECTED AND IN DECIDING WHAT CHANGES SHOULD BE MADE. THE SURVEY WILL BE CONDUCTED DURING 1984.
