ORDER FORM DATA IS NEEDED TO PROVIDE AN AUDIT OF THE TRANSFER AND PURCHASE OF SCHEDULE I & II CONTROLLED SUBSTANCES. DEA-222A REQUISITION FORM IS USED TO OBTAIN DEA-222 ORDER FORMS.
