Abstract: THE PURPOSE OF THIS FORM IS TO PROVIDE HUD WITH A UNIFORMED METHOD OF MONITORING THE ACTIVITIES OF COMMUNITY HOUSING RESOURCE BOARDS AND TO ASSURE HUD OF THE ADEQUATE USE OF GRANT FUNDS TO AVOID FRAUD, WASTE AND MISMANAGEMENT. THE RESPONDENTS ARE HUD APPOINTED MEMBERS OF VOLUNTEER FAIR HOUSING GROUPS.
