Abstract: THIS SURVEY IS USED TO PERIODICALLY OBTAIN CONSUMER INPUT ON SATISFACTION WITH SERVICES PROVIDED. RESULTS ARE USED TO MONITOR FACILITY PERFORMANCE ACCORDING TO (TENTATIVE) STANDARDS ESTABLISHED WITHIN DM&S.
