Abstract: IN ORDER TO PARTICIPATE IN THE MEDICARE/MEDICAID PROGRAM AS A CORF PROVIDERS MUST MEET FEDERAL CONDITIONS FOR COVERAGE. THE CERTIFICATIO FORM IS NEEDED TO DETERMINE IF PROVIDERS MEET AT LEAST PRELIMINARY REQUIREMENTS.
