WHEN THIS PACKAGE IS RESUBMITTED BEFORE THE FINAL RULE IS ISSUED, EPA SHOULD IDENTIFY THE MAJOR COMMENTS RECEIVED DURING THE PUBLIC COMMENT PERIOD AND ANY CHANGES MADE TO THE INFORMATION REQUIREMENTS IN THE RULE AND THIS ICR. PLEASE CITE OMB COMMENT NUMBER 2040-0080. IN ADDITON, EPA SHOULD ADDRESS THE FOLLOWING POINTS: 1. HOW WILL EPA REVISE PERMIT CONDITIONS FOR NEW SOURCES ONCE THE PLANT IS IN OPERATION AND EPA FINDS THAT IT'S NOT PROVIDING "ADEQUATE CONTROL"? HOW IS ADEQUATE DEFINED AND WHAT ADDITIONAL INFORMATION MIGHT EPA REQUIRE? 2. ITEMS V-D, E, F ARE DISCUSSED ON PAGE 6 OF THE JUSTIFICATION STATEMENT BUT ARE MISSING FROM THE 2-d INSTRUCTIONS. 3. WHY IS THE SMALL BUSINESS EXEMPTION LIMITED TO CERTAIN COAL MINES AND BUSINESSES WITH LESS THAN $100,000 IN GROSS PROFITS? WHAT IS THE BASIS FOR $100,000? COULD THE NUMBER BE DIFFERENT WITHOUT SIGNIFICANT ENVIRONMENTAL PROBLEMS? ARE NONPROFIT ORGANIZATIONS COVERED (E.G., SMALL UNIVERSITIES)? 4. PLEASE EXAMINE MORE CAREFULLY THE ACCOUNTING FOR BURDEN HOURS BETWEEN OLD AND THESE NEW ICRs AND SUBMIT ANY CORRECTION CHANGES SHEETS AS NECESSARY. FORM 2D MUST BE FILED BY ALL NEW SOURCES AND NEW DISCHARGERS TO OBTAIN NPDES PERMITS. BOTH FORMS ARE SUPPLEMENTAL TO NPDES FORM 1. EPA USES THE APPLICATION INFORMATION TO ESTABLISH PERMIT CONDITIONS FOR THESE DISCHARGERS. 00000
