Terms of Clearance: Practical utility of extensive questionnaire not adequately demonstrated, survey objectives seem to go far beyond the study of payment methods, e.g. assessing feasibility and impact of shift to direct payments, setting up better departmental controls, determining training needs of smaller companies, voluntary collection appears inappropriate for this type of inquiry. Objectives of the survey should be formulated more explicitly and precisely.
