Abstract: THIS FORM IS NECESSARY TO DOCUMENT SHIPMENTS OF BEER AND WINE INTO THE U.S. FROM PUERTO RICO. THE FORM DESCRIBES THE SHIPMENT FOR TAX PURPOSES, THE TAX TO BE IMPOSED AND ITS CALCULATION AND THE VERIFICATI OF THE TAX COMPUTATION WHICH IS CERTIFIED BY BOTH PUERTO RICO AND U.S. OFFICIALS.
