Abstract: THIS FORM IS USED TO CONTACT CHARACTER REFERENCES OF A SUBJECT OF A PERSONNEL SECURITY INVESTIGATION (PSI) WHEN ATTEMPTS TO MAKE PERSONAL CONTACT ARE UNSUCCESSFUL.
