THE MOST EFFICIENT PROVISION OF HOSPICE CARE. JCAH HAS BEEN CHOSEN
TO CONDUCT A SURVEY TO PROVIDE HCFA WITH INFORMATION ABOUT HOSPICES
NOT CURRENTLY PARTICIPATING IN MEDICARE AND THEIR ABILITY TO MEET
MEDICARE CONDITIONS OF PARTICIPATION.

the collection of information encompassed by this request complies
