Abstract: THE INFORMATION COLLECTED VIA THE USE OF THIS FORM MAY ENABLE AN INDIVIDUAL TO MEET THE INSURED STATUS REQUIREMENTS FOR ENTITLEMENT TO SOCIAL SECURITY BENEFITS USING CURRENT YEAR SELF-EMPLOYMENT INCOME (SEI). IT WILL ONLY BE COLLECTED IF IT CAN BE USED TO ENTITLE A PERSON TO BENEFITS SOONER.
