Abstract: THIS INFORMATION COLLECTION REQUIREMENT IS MANDATED BY SECTIONS 515(B)(10) AND (13) OF P.L. 95-87. THE INFORMATION IS USED TO ENSURE COMPLIANCE WITH STATUTORY PERFORMANCE STANDARDS FOR THE CONSTRUCTION AND OPERATION OF DAMS COMPOSED OF WASTE MATERIALS AND TO MINIMIZE DISTURBANCE TO THE QUALITY AND QUANTITY OF SURFACE WATER SYSTEMS.
