IF THERE ARE ANY TAX BENEFITS TO BE DENIED, SCHEDULE A (FORM 5713) OR SCHEDULE B (FORM 5713) AND SCHEDULE C (FORM 5713) MUST ALSO BE FILED TO SHOW HOW THE TAX BENEFITS AND AMOU TO DENIED WERE FIGURED.
