Abstract: THE ADDENDUM COLLECTS INFORMATION REGARDING EX-SPOUSES ELIGIBILITY FOR SURVIVOR BENEFITS IN CASES OF DEFERRED RETIREMENTS UNDER THE CIVIL SERVICE RETIREMENT SYSTEM.
