Abstract: THIS FORM IS DESIGNED FOR USE BY ELIGIBLE ANNUITANTS TO PROVIDE A SURVIVOR ANNUITY BENEFIT FOR ELIGIBLE FORMER SPOUSES UNDER PUBLIC LAW 98-615. THIS LAW ALLOWS ANNUITANTS TO PROVIDE A SURVIVOR BENEFIT FOR THE FORMER SPOUSE FOR THE FIRST TIME. THIS ELECTION MUST BE MADE WITHIN 18 MONTHS AFTER THE DATE OF ENACTMENT OF THE LAW OR MAY 8, 1986.
