Abstract: FORM FS-299 IS USED BY PERSONS REQUESTI EXTENSION OF A PASSPORT THAT WAS LIMITED INITIALLY FOR INSUFFICIENT CITIZENSHIP EVIDENCE. IT IS ALSO USED BY PERSONS REQUESTING THAT THE THE PASSPORT BE AMENDED TO REFLECT A CHANGE OF NAME. THE RECORD OF THESE SERVICES BECOMES PART OF THE PERSON'S PERMANENT PASSPORT FILE IN THE STATE DEPARTMENT.
