Abstract: FORM 6458 IS USED TO MAKE OR REVOKE VARIOUS ELECTIONS AND CERTIFICATIO UNDER THE CRUDE OIL WINDFALL PROFIT TAX ACT. THE IRS USES FORM 6458 TO RECORD PERSONS WHO ARE CLAIMING EXEMPTION FROM OR REDUCED RATES OF THE WINDFALL PROFIT TAX, AS WELL AS OTHER ELECTIONS AND REVOCATIONS.
