Abstract: THE INFORMATION COLLECTION IS NEEDED TO SUMMARIZE THE EMPLOYERS MONTHLY PAYROLL DEDUCTION ACTIVITY FOR VA LIFE INSURANCE PREMIUMS. IT IS USED BY THE VA FINANCE ACTIVITY TO VERIFY THAT THE EMPLOYER HAS FORWARDED A COMPOSITE PREMIUM CHECK FOR THE PROPER AMOUNT.
